Our partners and program funders are vital to CHC/ACT.

Grants, financial support, and program partnerships bring improvements to community health in Connecticut.

CT Health Foundation

CHC/ACT works with the CT Association for Community Action, HES-CT, and Southwestern AHEC to advocate for reimbursement of Community Health Worker services through Medicaid. The work includes outreach to existing CHWs, community-based organizations, and others to: educate about the legislative process, educate about the need for/value in CHW reimbursement, and promote advocacy throughout the 2025 legislative session.

HRSA programs provide equitable health care to people who are geographically isolated and economically or medically vulnerable.

State & Regional Primary Care Associations

State and Regional Primary Care Association (PCA) Cooperative Agreements provide education and technical assistance to existing and potential health centers in their states and regions.

As Connecticut’s PCA, CHC/ACT helps increase access to comprehensive primary care, foster a workforce to address current and emerging needs, enhance emergency preparedness and response, and advance clinical quality and performance.

CHC/ACT convenes numerous workgroups comprising health center staff and experts, to collaborate and share best practices, towards better health for all.

Health Center Controlled Network

Health Center Controlled Networks (HCCNs) support health centers in leveraging health information technology (IT) and data to deliver high-quality, personalized, and comprehensive primary health care, with a specific focus on improvements in:

  • Clinical quality
  • Patient-centered care
  • Provider and staff well-being

CHC/ACT’s HCCN brings together 20 Community Health Centers across Connecticut and Rhode Island for a powerful, impactful network of experts improving health care quality and outcomes.

Ryan White HIV/AIDS Program Part D

Coordinated HIV Services and Access to Research for Women, Infants, Children, and Youth (RWHAP Part D WICY)

CHC/ACT’s RWHAP Part D WICY program provides family-centered health care services in an outpatient or ambulatory care setting for low income WICY with or impacted by HIV.

The DSS delivers and funds a wide range of programs and services as Connecticut's multi-faceted health and human services agency.

Medicaid Outreach

Through funding from the State Department of Social Services, CHC/ACT helps fund outreach/enrollment efforts for Medicaid, CoveredCT, and Access Health CT. CHC/ACT also works with outreach workers at Connecticut’s health centers to ensure they are certified to conduct enrollment, share outreach/enrollment best practices, troubleshoot individual cases, identify and work to resolve systemic issues, and act as a liaison with relevant state agencies.

The CT Office of Health Strategy crafts policies for better outcomes, curbing healthcare cost growth across all sectors.

CoveredCT

CHC/ACT employs three Community Health Workers through this program to travel to health centers and community events to assist in outreach, education, and enrollment in the CoveredCT program.
