Job Description
Triage Nurse
The Triage Nurse is primarily responsible for managing urgent and emergent patient calls, in addition to, performing professional nursing functions to assess the severity of walk-in patients requesting healthcare services. The Triage Nurse functions as a member of the healthcare team, conducting duties such as; patient medication refills, managing providers’ schedules, patient advice, and other related tasks as needed.
Responsibilities:
- Identifies patient care requirements.
- Manages nurse-directed patient telephone calls and from walk-in patients requesting health services.
- Evaluates and assesses patient’s healthcare needs based on objective and subjective patient information.
- Coordinate and schedule same-day and walk-in appointments on the providers’ schedules.
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments
- Manage prescription refills per protocol.
- Utilize standing nursing orders and triage protocols appropriately.
- Accurately documents symptoms/complaints, nursing assessments, advice provided and patient/caller response in patient and department records.
- Resolves patient problems and needs by utilizing multidisciplinary team strategies as appropriate and assists in the development of case management metrics to identify opportunities to improve care.
- Maintains safe and clean working environment by complying with procedures, rules, and regulations.
- Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
- Contributes to team effort by accomplishing related results as needed.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
- Case management and other duties as assigned by Clinical Services Manager.
- Complies with all infection control policies and procedures outlined by the organization in accordance with professional, state, and federal guidelines.
Requirements:
- Graduate of accredited Nursing program
- CT State Registered Nurse license
- 3 years of varied clinical experience strongly preferred
- Bi-lingual, preferred
