Job Description
- Initialize and complete timely referrals while maintaining ongoing tracking and appropriate documentation
- Ensure accurate registration, including patient demographic and current insurance
- Assemble patient’s clinical background information and referral needs
- Problem solve potential issues related to the health care system, financial, or social barriers
- Act as point of contact for patients and families, allowing direct access for questions
- Establish and maintain relationships with identified service providers
- Advocate on patient’s behalf with insurance carriers to ensure approval of necessary supplies / services

Education:
- High school or equivalent (Required)
Experience:
- Community Health Center and/or Medical office: 1 year (Preferred)
