Job Description
Fast-paced community health center seeks:
- a Child and Adolescent Licensed Clinical Social Worker responsible for clinical service provision and clinical case management within the Pediatric Medical Department.
Some responsibilities include of this LCSW Child and Adolescent Behavioral Health Clinician include:
- Providing initial behavioral health evaluation in an outpatient clinic
- Performing assessment utilizing current DSM diagnostic criteria, evaluation including psychosocial assessment, and mental status exams for adolescents and families
- Identifying patient problems; completing Initial and Master Treatment Plans to ensure that patients receive evidence-based treatment
- Case presentation within multidisciplinary clinical team meetings
- Conducting direct and individual, group, and family therapy as clinically indicated.
- Utilizing telehealth technologies as appropriate to provide care to clients.
- Participates in coordination of care planning with local providers, state agencies, schools and managed care agencies as necessary.
- Provides clinical documentation to external providers from various community-based disciplines when appropriate.
- Documenting findings and progress toward patient goal attainment
- Providing appropriate discharge planning and continuity of care in conjunction with a Behavioral Health Case Management team
- Provides clinical case management as needed to Pediatric Medical Department.
- Provides crisis intervention as needed to Pediatric Medical Department.
- Participates in weekly clinical supervision, weekly staff meetings, and administrative supervision as required.
- Coordinate utilization authorization management functions with third-party payers. (obtain insurance pre-auths at time of Intake/Assessment and upon expiration of pre-auth period).
- Monday – Friday, 8:00 a.m. – 4:30 p.m.
- No Weekends
- No After Hours or On-Call
