Registered Nurse Home Visitor

Overview:

Family Centers is seeking an experienced and motivated Registered Nurse for our Parents as Teachers program in lower Fairfield County, CT.

The Parents as Teachers program is a home visiting program that utilizes an evidence-based model that focus on target areas of intervention, namely nurturing parenting, healthy families, parent life outcomes, school readiness, healthy development, and emotional security.  The program provides home visits and is delivered from the third trimester of pregnancy until the child reaches the age of five.

The Role:  

The Nurse Home Visitor will provide support postnatally via in-person and virtual visits, as well as telephone support via a warm line.  The incumbent will need to build trusting, effective relationships with the families they serve, and serve as a resource to the families and colleagues.

Requirements: 

• Active CT RN license
• Prior experience with pediatrics and/or working with vulnerable families
• Access to vehicle with ability to travel locally (Stamford, Greenwich, Darien and New Canaan)
• Strong communication skills (verbal and written)
• Bilingual (Spanish/English) preferred
