Physician Assistant or Family Nurse Practitioner

Family Centers seeks an experienced Physician Assistant (Primary Care) or Nurse Practitioner (FNP or PNP) to deliver comprehensive primary medical care to UC residents in an outpatient clinic.

Family Centers, non-profit organization offering health care, education and human services to children, adults, and families living in Lower Fairfield County. More than 200 professionals and 2,500 volunteers collaborate to provide our communities with a wide range of responsive and innovative services.

The Role:

The Primary Care Provider will be responsible for intake assessments for UC residents, diagnosis and treatment of common and chronic illnesses, referrals, vaccines, screenings, promoting optimal health and preventative care. The Primary Care Provider will collaborate with UC program team members, be responsible for the medication management and medical case management. This flexible full time opportunity that will require some evenings and weekend hours.

Education and Experience:

Master’s or post-Graduate’s degree from an accredited program
Current active license to practice in the State of Connecticut
Minimum of 2+ years related experience and/or training is preferred
Demonstrated aptitude in providing medical services to under-served populations.
Experience in treating the Pediatric population is preferred
Excellent multitasker, leadership, and team oriented
Bilingual (Spanish) is required.
Medical malpractice insurance (in addition to that offered by the agency)
Current active DEA license
Infectious Control Certified a plus
