Physician Assistant or Family Nurse Practitioner

Overview:

Family Centers Healthcare seeks a bilingual, experienced, energetic, and compassionate Physician Assistant (Primary Care) or Nurse Practitioner (FNP or PNP) to work as part of a dynamic, multi-disciplinary team to deliver comprehensive primary medical care to our patients in the community.

Family Centers, a non-profit organization offering health care, education and human services to children, adults, and families living in Lower Fairfield County. Our health center, Federally Qualified Health Center (FQHC), provides primary healthcare, dental and mental health services to those living in this region.

The Role:

The Primary Care Provider functions as a health team member in meeting the objectives of the program. Incumbent will be responsible for the diagnosis and treatment of common and chronic illnesses, planning of periodic health assessments, vaccines, screenings, promoting optimal health and preventative care. Additional responsibilities include community referrals and monitoring of medication administration by certified staff and other regulatory compliance documentation.

 

Education and Experience:

Master’s or post-Graduate’s degree from an accredited program
Current active license to practice in the State of Connecticut
Minimum of 2+ years related experience and/or training is preferred
Demonstrated aptitude in providing medical services to under-served populations.
Experience in treating the Pediatric population is preferred
Excellent customer service/ communication skills.
Knowledge of infection prevention and control; Infectious Control Certified a plus
Excellent multitasker, leadership, and team oriented
Bilingual (Spanish) is required.
Demonstrated ability to utilize computer technology and to willingness learn.
Knowledge in using EPIC electronic medical record system a plus.

Medical malpractice insurance (in addition to that offered by the agency)

Current active DEA license
