LPN- Behavioral Health


Reporting directly to the Behavioral Health Coordinator, the Behavioral Health Nurse is responsible for the daily support
of the Psychiatrists by performing the following functions:

· Checking
patient’s vital signs,

· Acquiring authorization
for treatment, and prior authorization for medication

· Coordinating care
in collaboration with the clinical team

· Communicates
changes to medication and treatment plan to appropriate clinical staff and
medical providers

· Assists with case
management needs of the program.

· Will develop
collaborative working relationships with Psychiatrists, clinicians, physicians,
and staff, both internal and external to the department.

· Will meet minimum
competency requirements for specific aspects of their position as developed by
the Behavioral Health Coordinator and Psychiatrists.

· As a key member
of the MAT team, will participate in the MAT monthly meeting and care team is

· Will participate
on the Integrated Care Committee and assist in integration activities and act
as a liaison between behavioral health and medical staff to enhance communication


Key Job Requirements:


1) Under the direction of the Behavioral
Health Coordinator and Psychiatrist, responsible for monitoring patient’s vital
signs, coordinating care with clinicians and medical providers, and assisting
in MAT and integrative care committee with Psychiatrist oversight.


2) Under the management of the Behavioral
Health Coordinator, perform various administrative functions related to the day
to day operations of the unit. Those functions include but may not be limited
to: coordination of referrals, medication
reconciliation, monitor and coordinating lab results, performing AIMS exam, completion
of authorization for BH treatment, coordination of care for clients, ensuring
accurate and appropriate transportation is available for clients, administering
evidenced based assessments for clients, tracking patient outcomes,
pre-authorizations for medication, ensuring proper documentation gets to
Psychiatrists for medication refills, communicates with patients about
medication refill requests, communicates with Pharmacy when needed, and communicates
changes to medications with patients under the guidance of Psychiatry.


3) Supports BH PSR by assisting with
patient’s needs, responding to crisis, obtaining clinical support, answers
questions about medication, communicates patient’s needs to clinical staff, and
other tasks as directed by Behavioral Health Coordinator or Psychiatrists.


4) Ensures that Behavioral Health
clients are assessed for social determinants and addresses those needs or makes
a referral as appropriate. Maintains confidentiality. Displays a positive, interested and motivated
attitude with members of the clinic and medical staff and when representing the
clinic to outside contacts and/or review agencies. Shows courtesy and concern when contacting
patients or their representatives.


5) Documents all patient information
into the medical chart and follows Psychiatrists written orders.


6) Participates in all Behavioral
Health Meetings, including but not limited to Behavioral Health Department Meeting,
Clinical Meeting, Staff Meeting and ongoing consultation meetings with

Physical Effort/Environment:


Physical Demands (More than 4 hours per day)-Requires walking,
bending, sitting, standing, writing, reading, telephone use, data input into
computer, and high level of audio and visual acuity. Moderate physical
activity. Requires handling of
average-weight objects up to 25 pounds.


Mental Demands- Ability to cope with continually changing priorities
under potentially stressful conditions.


Manual Dexterity Required-Ability to use a keyboard, computer, and telephone;
use of medical equipment requiring fine motor skills.


Work Schedule: Full-time:
40 hours per week


Education and Experience Requirements:

Graduate of
an accredited school of nursing. Current, unrestricted state license as a
Licensed Practical Nurse in the state of Connecticut. Current CPR certification.
Demonstrated proficiency in clinical assessments, documentation and compliance
with nursing care and policies and procedures. Experience working with a wide range of patients preferred. Some history of working with psychiatric
patients needed. Knowledge of social,
psychological and physical factors of mental health disorders necessary. Experience working with MAT patients not
needed but appreciated. Must have
working knowledge of nursing principles, procedures and techniques.


Communication Skills:


good verbal skills to communicate with patients and staff members daily. Good listening skills to understand both
patients and staff. Good penmanship and
clear concise writing skills. Has a pleasant and respectful attitude when:
communicating with coworkers, patients and internal/external clients; accepting
direction and delegated tasks; and receiving constructive criticism. Oral and
written proficiency in English; bilingual with fluency in Spanish preferred.
