Licensed Clinical Social Worker (LCSW) – Intensive Outpatient Program (IOP)
Fast-paced community health center seeks:

• A Licensed Clinical Social Work (LCSW) responsible for clinical service provision to adult patients within the Department of Behavioral Health

Some responsibilities include of this LCSW include:

• The ability to demonstrate good reasoning and decision making skills and be able to work collaboratively in a team approach

• Providing initial diagnostic assessment utilizing current DSM diagnostic criteria, evaluation including psychosocial assessment, and mental status exams for adults
• Identifying patient problems; completing Initial and Master Treatment Plans to ensure that patients receive evidence based treatment
• Case presentation within multidisciplinary clinical team meetings
• Conducting direct, individual, group, couples, and family therapy sessions
• Conducting and facilitating psycho-educational and psychosocial group therapy sessions
• Documentation of findings and progress toward patient goal attainment to meet audit compliance standards
• Providing appropriate discharge planning and continuity of care
