HCCN Coordinator

POSITION SUMMARY: 

The HCCN Program Coordinator is primarily responsible for administration of project deliverables, assisting in the implementation of an annual workplan for the Health Center Control Network (HCCN) federal grant. The position will report to the Sr. Program Manager.

ESSENTIAL FUNCTIONS:

  • Assist and provide input in creating the annual HCCN workplan.
  • Collect, analyze, and monitor program data and narrative reporting to ensure deliverables of program contracts are met.
  • Monitor expenditures and maintain documentation of budget/program revisions, and program and financial expenditure reports.
  • Identify subcontractors and consultants as they pertain to organizational projects and programming.
  • Create contracts for work with subcontractors and consultants as it pertains to program guidelines.
  • Monitors the progress of numerous program activities to ensure timeliness and completion in line with program goals.
  • Disseminate information to FQHC staff and serve as point of contact for programmatic questions.
  • Assist with the development of grant applications to various funding agencies.
  • Manage and prioritize multiple projects, assignments, and deadlines for grant applications with various requirements.
  • Coordinate meetings and training for FQHC program staff and subcontractors in accordance with contract and program guidelines.
  • Sustain FQHC subcontractor, program partner, and vendor relationships as a representative of the agency.
  • Provide administrative support for consulting contracts with member health centers and other organizations.
  • Work collaboratively with both internal staff and external organizations.
  • Perform other department or agency-related duties or special projects as assigned.

 QUALIFICATIONS:

  • Experienced in HIT use in support of quality improvement related to patient care, finances, and operations at FQHCs.
  • Experience working with grant funding agencies, community partners, and individuals from the community effectively, clearly, and positively via phone, email, and in-person.
  • Must be detail-oriented, and able to work both as a team player and independently in a frequently changing environment.
  • Must have the ability to effectively speak before groups and present training to all types of audiences.
  • Must possess and maintain a valid Driver’s License and insurance and must successfully complete a motor vehicle history check. This position requires driving a personal vehicle on behalf of CHCACT. Travel (up to 30%) may be required to travel to visit member FQHC’s or attend meetings throughout the state of CT.
  • This position has no direct supervisory responsibilities.

EDUCATION AND/OR EXPERIENCE:  

Bachelor’s degree in appropriate field (health information management, management information systems, public health, health administration, or other health related field) or relevant experience.  Master’s degree preferred.  A minimum of 2-3 years of demonstrated skills and working in the fields of health, public health, or social service.  Federally Qualified Health Center experience also preferred.
