Job Description
Provides direct clinical social work services, including individual, group, and family therapy to LGBTQ+ community and general mental health population. Collaborate with an interdisciplinary team focused on the needs of the LGBTQ+ population, including primary care providers, and Infectious Disease physician. Performs outreach into the community to link LGBTQ+ identified patients to care. Performs case management services. Provides extensive documentation and care coordination. Participates in the Center’s quality assurance, performance improvement and risk management activities as required by Center policies and procedures or as assigned by Center leadership.
Duties and Responsibilities
Assessment:
- Demonstrates ability to compile and complete assessment information in documenting comprehensive and relevant case formulations, and treatment planning
- Demonstrates ability to compile and complete assessment information in documenting comprehensive and relevant case formulations, and treatment planning
- Demonstrates the ability to conduct ongoing risk assessments (i.e., SI, HI)
- Demonstrates professional experience with LGBTQ+ community, specifically advocating and training in transition process and documentation for the transgender population
- Demonstrates ability to develop treatment plans that include observable and measurable goals and modify as necessary
- Demonstrates the ability to integrate clients’ needs, strengths, and barriers to treatment
- Provides individual therapy services to patients
- Provides group therapy services to patients
- Provides family therapy services to patients
- Provides evidence-based treatment for LGBTQ+ community and families
- Provides extensive documentation of the case, including (based on program needs) Social History, progress notes and Treatment via E Clinical Works
- Provides supportive referrals and proper documentation for gender-affirming treatments, including letters of support
