To provide high quality health care to the patients of the Fair Haven Community Health Center in the primary care setting consistent with FHCHC policies and mission.
Duties and Responsibilities
Fair Haven Community Health Center is seeking a Registered Dietitian to oversee the nutritional well-being of our patients. The successful candidate will have a proven track record of conducting nutrition assessments, identifying patients at risk and creating diet plans and will be responsible for coordinating all phases of nutrition care including, monitoring, education and counseling of patients within the FHCHC setting. Typical duties include but are not limited to:
Provide dietary services in accordance with FHCHC guidelines, state and federal laws and regulations
Develop nutrition plans and implement interventions based on knowledge of patient’s current health
Complete monthly, quarterly and yearly assessments of clients and adapt nutrition plans as needed
Implement personalized, age-specific and culturally appropriate nutrition strategies
Deliver client nutrition recommendations to physicians, nurses and aides
Provide nutrition education and counseling for patients
Masters degree from an accredited college or university in nutrition, dietetics, or dietetics and food administration or Bachelors in any of the above and Masters in a related field is required. Experience with one-on-one and group nutrition counseling and the cultural competence and sensitivity to work with a diverse population is also required. CT state dietitian-nutritionist certification or certification eligible with certification within 6 months of hire.
Must work well independently to prioritize patient care, and work well as part of the healthcare team. Must be able to use computer software systems including, but not limited to, word processing, spreadsheet, presentation, and nutrient analysis programs, as well as varied electronic medical records systems.
