|Family Centers Healthcare seeks an experienced, energetic, and compassionate Advanced Practical Nurse Practitioner (FNP or PNP) or Physician Assistant (Primary Care) to work as part of a dynamic, multi-disciplinary team to deliver comprehensive primary medical care to our patients at in the community. This is a part time role.
Family Centers, non-profit organization offering health care, education and human services to children, adults, and families living in Lower Fairfield County. Our health center, Federally Qualified Health Center (FQHC), provides primary healthcare, dental and mental health services to those living in Greenwich Public Housing communities and the surrounding neighborhoods.
The Primary Care Provider functions as a health team member in meeting the objectives of the program. Incumbent will be responsible for the diagnosis and treatment of common and chronic illnesses, planning of periodic health assessments, vaccines, screenings, promoting optimal health and preventative care. Additional responsibilities include community referrals and monitoring of medication administration by certified staff and other regulatory compliance documentation.
