Job purpose
The Director of Nursing at Fair Haven Community Health Care is a unique opportunity for a nursing leader to shape front-line care delivery for a high-need population across Greater New Haven while integrating with a preeminent academic medical center. The Director oversees the nursing and medical assistant staff across all sites, and is responsible for the workplace culture, team morale and wellbeing, clinical quality, and clinical safety of the Nursing Department. The Director is a key liaison with Yale-New Haven Hospital’s Nursing Department and Yale School of Nursing, which staff and support the clinical care and education at some of FHCHC sites. The Director recruits, retains, organizes, and manages the Nurse Managers at all sites. The right leader will demonstrate energy, compassion, excellence in clinical quality and safety, an equity lens, and a collaborative approach to cultivating a growing Nursing Department.
Duties and responsibilities
Reporting to the Vice President of Clinical Affairs, the Director of Nursing manages all the services provided by the nursing personnel of FHCHC. Typical duties include but are not limited to:
- Functions as a member of FHCHC’s clinical leadership team including participating in meetings specified by the VP of Clinical Affairs.
- Represents FHCHC in discussions with community stakeholders that pertain to the front-lines of care.
- Develops and maintains strong working relationships with clinical leaders of other provider organizations (e.g. Yale-New Haven Hospital).
- Develops and implements innovative programs and workflows, policies, practices and/or guidelines to meet and/or exceed quality, access, patient/staff safety and efficiency targets.
- Develops and maintains written objectives, organizational plans, and deliverables for nursing services; assists in the development of patient care policies
- Ensures nursing department is compliant with federal, state and local regulations
- Serves as a clinical leader in various projects and forums and participates in strategic planning
- Facilitates integration of nursing services with other patient care services in the facility, and ensures nursing staff patterns are implemented
- Assists in the recruitment, development and orientation of nurse managers and nursing staff
- Identifies educational and clinical needs of the nursing staff and schedules in-services to address needs
- Ensures care of patients through health promotion, maintenance, restoration and prevention of infection, accident and injury; monitors quality and appropriateness of patient care
- Participates in oversight of immunization tracking and lab diagnostic tracking
- Promotes the role of the patient and practice within a Patient Centered Medical Home
- Serves as an agent for change by constantly seeking opportunities to shape the environment
- Provides nursing care services as needed, including but not limited to administering medications; assesses patient’s response to treatment and generalized nursing care
- Oversee the Laboratory and ensures the overall technical operation to ensure excellent and quality services
- Organizes and facilitates regular meetings of nursing department
Qualifications
RN required; BSN or MSN preferred; three to five years of ambulatory care experience, preferably in a federally qualified health center setting, is also required. Prior nurse manager experience and leadership skills including the ability to motivate & supervise others is required.
Knowledge and experience with primary care setting, including program development and quality assurance methodology; familiarity with clinical audits and the Lean methodology are highly preferred. The selected candidate will have excellent communication skills, both written and oral and experience with electronic medical record
