COVID Community Outreach Manager
POSITION SUMMARY
The Manager is responsible to oversee and deliver COVID testing and vaccination programming in the community and at HWC center locations and works closely with an integrated care team including, medical/psychiatric providers, medical assistants, other nurses, clinicians, dental and support staff and other specialty services to ensure the effective and efficient delivery of primary health care, treatment and services to all patients.
EDUCATION AND EXPERIENCE/QUALIFICATIONS
Bachelor’s degree in a related field with a minimum of 2 years providing administrative oversight and supervision to staff who provide services to patients from diverse economic and cultural backgrounds, as well as exceptional communication, interpersonal and organizational skills are required. Outpatient primary care settings and/or community outreach experience preferred. Bi-lingual English/Spanish preferred.
Must have a drivers license and access to reliable transportation.
LOCATION
FQHC in Hartford, CT
Position will require travel to all Wheeler health center sites (Hartford, New Britain, Waterbury, Bristol) and other community locations as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Oversees the delivery of COVID testing and vaccination programming in the community and at HWC locations.
Under the direction of the VP of Nursing, oversees Test Site staff and when necessary, communicates directly with State Agencies and community partners at pop up test site locations.
Attends any agency, local and State coordination meetings related to COVID.
Provides oversight at community pop up test sites and HWC locations as needed.
As necessary, provides testing to patients, either at a health center location or at a community setting.
As necessary, works directly with patients to obtain demographic and symptom related data in order to register for testing and/or vaccination.
As necessary, contacts all patients in a timely fashion in order to furnish results, completes all required EHR and state required documentation and if requested prints results and delivers via e-mail, fax or otherwise to patients.
Serves as liaison to medical providers and other staff.
Protects patients and employees by adhering to infection-control policies and protocols, not limited to hand hygiene and personal protective equipment usage.
Must be able to pass a respirator fit test at time of hire and on an annual basis.
Participates with regular staff meetings focused on coordinating care within an interdisciplinary team.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological
approaches, skills, and techniques that reflect an understanding of culture.
