We were made for the community, by the community, to offer a full spectrum of services to include medical, behavioral health care, dental care, a full-service pharmacy and more. Community Health Services, Inc. is seeking a Community Outreach Specialist to support our efforts to identify groups and individuals in the community who could benefit from the clinical services we offer and bring them into care.

The successful candidate will:
  • Have a bachelor’s degree in counseling, social work, health care or a related field OR relevant work experience in lieu of formal education

  • Have 2-3 years of experience working with underserved populations with a particular emphasis on reaching populations outside of traditional settings

  • Be comfortable conducting field work in a variety of urban and rural environments, including on evenings and/or weekends

  • Possess an understanding of and have demonstrated success providing basic education regarding the most common chronic conditions in under-served populations

  • Have a deep understanding of the social determinants of health and their impact on health outcomes

  • Be able to communicate effectively and clearly with individuals across a broad range of racial, ethnic and socioeconomic statuses

  • Have a track record of producing reliable, high quality results with minimal direct oversight

  • Speak and write fluently in both Spanish and English

  • Thrive in an environment with tight deadlines and changing priorities where everyone is expected to do a lot with a little

  • Demonstrate alignment with CHS Core Values and a high level of personal accountability

  • Be willing to have fun at work

  • Demonstrate compassion and respect for individuals in challenging situations

  • Possess a valid Connecticut driver’s license and a reliable vehicle

  • Have proven success handling confidential information

  • Be able to use Microsoft Office products, including Excel

  • Demonstrate alignment with the organizational mission
