APRN – School Based

Optimus Health Care is a Federally Qualified Health Care Center with sites in Bridgeport, Stamford, and Stratford, CT. We are the largest provider or primary health care services in Fairfield County, proudly serving all patients regardless of their income, race, or insurance status.

We are presently recruiting for an Advanced Practice Registered Nurse or Physician’s Assistants to work in our School Based Health Center site in West Haven, CT, providing primary care. This program offers health education, oral health, mental health, and other services for school-aged patients.

This is a 35 hour per week, benefits eligible position and follows the School Calendar. This role participates in a Saturday rotation at a non-school based health center and telephonic call.

About this Position:

  • Experience and interest in providing care to children & Adolescents
  • Collaborate with School Administrators
  • Obtain complete medical history & physical data for patients
  • Determine appropriate diagnostic procedures as needed
  • Perform complete physical exams & record results in EMR
  • Develop quality control/assurance standards
  • Educate patient and families regarding medications, health, and illness protection.

Qualifications:

  • Graduate of an accredited school of nursing and successful completion of an approved Nurse Practitioner program.
  • 1-3 years’ experience in the area of pediatrics
  • Active CT FNP Licensure or in process
  • Board Certified
  • Bi-lingual helpful (Proficiency in Spanish or Portuguese a Plus)
