APRN (Mid-Level)

Summary
The Advanced Practice Registered Nurse practices medicine under a collaborative agreement with a departmental physician, with both disciplines practicing within the scope and boundaries of State regulations. Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC. Promotes and maintains health by providing services under the supervision of the Department Chief/Physician.

Essential Duties and Responsibilities

  • Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine.
  • Maintains confidentiality in regards to patients, staff and program matters at all times.
  • Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care, as may be indicated.
  • Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary.
  • Educates patients on preventive health care and chronic disease management.
  • Treats patients and staff respectfully, courteously, appropriately.
  • Maintains safe and clean working environment by complying with procedures, rules and regulations.
  • Protects clients and employees by adhering to infection-control policies and protocols.
  • Completes charting of notes, forms and coding in a timely and efficient manner. Enters appropriate legible notations, documents encounters appropriately.
  • Attends/participates and contributes to medical staff/departmental meetings.
  • Participates in trainings as required by the Department Chief.
  • Performs other related duties as assigned.

Qualifications

Education: A nurse practitioner must have an MSN degree and current CT RN/APRN licensure. Must have current Basic Life Support/CPR Certification.

Experience: Nurse Practitioner or Certified Nurse-Midwife with work experience and a knowledge base to perform the duties. One (1) year of experience as a health care provider preferred; however, new graduates are welcome.

Knowledge/Abilities: Ability to provide comprehensive, preventive, and therapeutic health care based on professional knowledge, clinic guidelines, standing orders, and specific health background of patients. Professional knowledge of medical theory and practice. Knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and management of acute and chronic patient/family problems. Knowledge and management of principles and practices including optimum use of staff and all other available resources.

The work involves sitting, standing, walking, bending, and occasional slight lifting. The employee occasionally lifts and/or moves up to 50 pounds, when transferring a patient from chair or wheelchair to exam table and turning patients for examinations. Must be able to respond when needed by prompt appearance at a required location. Must be able to work under physically and emotionally stressful conditions. The employee is frequently exposed to communicable diseases. Safety precautions for certain tasks such as blood drawing are observed according to organizational policy and procedures.

Must provide evening, weekend, and satellite coverage when necessary.
