Description

In collaboration with the UCFS physicians and team members, the Nurse Practitioner is responsible for providing quality patient centered primary care services for patients including comprehensive physical examinations, routine and appropriate sick care assessments and interventions. Develop and implement patient management plans, records, and progress notes per agency policies and procedures and approved protocols developed within the guidelines of the Connecticut Nurse Practice Act. Perform therapeutic procedures such as but not limited to; injections, immunizations, suturing, wound care, instruct and counsel patient and families regarding adherence with therapeutic regimen, normal growth and development and health promotion recommendations. Coordinate care with other providers of care and fosters patient advocacy by interfacing with community resources. Establishes and maintain patient continuity of care. Maintains and models excellent internal and external customer service.

  • Within the Scope of Practice of APRN, provides primary health care, including preventive care services, for patients including but not limited to:
  • Establishing a patient data base by obtaining an appropriate health, developmental and psychosocial history
  • Conducting a physical examination including ordering /performing /interpreting screening and diagnostic procedures and tests and provides follow up based on evidenced based treatment guidelines
  • Providing treatment according to accepted agency standards and protocols
  • Assisting in the management of referred and/or complex problems
  • Documenting assessment findings, treatment recommendations and plan for continuing care in the clinical record on a timely basis
  • Performs minor office procedures i.e., dressing changes, ambulating treatments and diagnostics in accordance with scope of practice
  • Makes referrals (internal and external ) and collaborates with physician, other agency providers, clinical support staff, administrative staff and community resources in meeting patient and family needs
  • In collaboration with patient and families, provides health promotion, education and counseling for patient/families based on patient need and self-management goals and focused on health risk reduction
  • Maintains accurate, legible medical records and returns all assigned charts or patient data (lab reports, consultations, etc.) to medical records in a timely fashion
  • Responds to telephone calls from patients and documents follow-up in a timely basis
  • Demonstrates knowledge and application of principles of fire, safety, infection control, and OHSA guidelines
  • Maintains knowledge of and acts in accord with third party payor requirements
  • Prescribes medicines for common primary care problems including timely renewal of medications for chronic, stable conditions, obtains consultation as appropriate when prescribing treatment for complex medical conditions, person with multi-system failures and or conditions that are resistant to treatment or when patients exhibit untoward or unusual response to treatment
  • Obtains physician consultation for prescription of schedule II or III controlled substances
  • Maintains productivity standards and assists other staff when time permits to assure smooth functioning of the primary care program
  • Adhere with current UCFS policies and procedures
  • Maintain ongoing education through completion of required continuing education; demonstrate efforts to maintain and improve job specific competencies and incorporates new knowledge into practice
  • Demonstrates positive communication skills when working with others; provides care consistent with customer satisfaction focus of care
  • Provides direction for medical assistant and other team members in meeting patient needs.

Requirements

  • Advanced Practice Registered Nurse Licensed to practice in the State of Connecticut
  • Prefer one to three years’ experience (Ambulatory care experience preferred)
  • Masters of Science Nursing or equivalency
  • Valid Driver’s License
