Integrating Community-Based Organizations with Health Care Providers

The Connecticut Health Foundation seeks to support integration between clinical care providers and community-based organizations in up to two Connecticut communities. The Connecticut Health Foundation will provide seed funding and technical assistance to collaboratives of at least three community-based organizations in each of the communities.

Integrated Behavioral Health Services Supplemental Funding

HRSA’s fiscal year (FY) 2019 Integrated Behavioral Health Services (IBHS) funding opportunity is a non-competitive supplement for organizations currently receiving Health Center Program operational grant (H80) funding. IBHS funding will help health centers increase access to high quality, integrated behavioral health services, including the prevention or treatment of mental health conditions and/or substance use disorders (SUDs), including opioid use disorder.

Connecticut Healthy Communities Project

The State of Connecticut Department of Public Health is seeking proposals to implement community-wide health promotion activities for the Connecticut Healthy Communities Project (CT HCP). Selected applicants will facilitate implementation of food service guidelines (FSGs) in community settings and worksites to increase the availability of healthier foods and coordinate with DPH to promote breastfeeding.
