The Community Health Center Association of Connecticut (CHCACT) is pleased to announce a training opportunity for community health workers on chronic diseases.
The registration fee is waived for the first 40 Connecticut-based users, courtesy of CHCACT.
MCD Public Health’s CHW Online Training Program is intended for community health workers and other individuals who assist clients in the prevention, management, and self-management of chronic conditions, such as cancer, high blood pressure, prediabetes, diabetes, and asthma.
This program complements CHW core competency training by providing individuals with added education, skill building and access to resources.

- Cervical Cancer
- Colorectal Cancer
- High Blood Pressure and Hypertension
- Prediabetes
- Diabetes
- Asthma
