Plans and carries out patient centered medical care including diagnosis and non-surgical treatment of diseases and injuries of internal organ systems for adults by performing the following duties in accordance with UCFS, AMA, NCQA and Backus Hospital Standards of Practice.
};;>- Directly supervises assigned medical students; Provides collaborative guidance to APRN as assigned; Provides input into the adult primary care clinical team performance evaluations (clinical team including NCC, RN, LPN, Medical Assistants)
ESSENTIAL DUTIES AND RESPONSIBILITIES –include thefollowing. Other duties may be assigned.
};;>- Provides patient centered medical care to clients/families of all ages in accordance with standards of professional ethics and practice, NCQA guidelines, AMA standards of care and WWBH hospital privileging standards and UCFS policies and procedures ; Meets documentation standards; analyzes records, reports, test results or examination information
to diagnose patient medical condition; ensures patient communication with clinical findings, recommendations and treatment plans
};;>- Provides continuity of care for clients and families through participation in on-call rotation; when appropriate, provides care in the acute care setting in which one has admitting privileges and provides back up coverage for other primary care providers; Makes referrals and follows up on education, counseling and self-care management strategies.
};;>- Ensures that special and holistic needs of clients/families are met through utilizing preventive health screening, treating common health problems, referring to specialists, utilization of primary care team members and other providers of care (i.e., dental behavioral health etc.), participating in interdisciplinary case conferencing and collaborating with other departments within UCFS and the community
};;>- Identifies and responds to potential and/or high-risk situations inclusive of notification of such agencies as Department of Elderly Affairs, Department of Children and Families as appropriate; participates in the development of health risk management protocols; Participates in the organizational plan for clinical operations, including but not limited to, identification of activities designed to respond to community health need, epidemiological trends, access issues, etc.
};;>- Participates in the development of a manpower plan for clinical operation that provides for the appropriate and efficient use of personnel to meet community and health problems
)> Participates in the development of the financial plan for clinical activities budget, including staffing and support plan, equipment needs projections with ongoing monitoring and evaluation; Meets productivity expectations.
J> Appropriately bills for services provided per 1CD-11 definitions and ensure that there is accurate, timely documentation to support the assigned billing code
)> Maintains current clinical knowledge of and adhere to third party payor requirements
)> Participates in regular staff meetings, chronic care meetings, provider meeting and clinical record review sessions to address clinical issues, new protocols, changes in procedures, review of infection control issues, etc.
)> Meets health care outcome expectations as identified in the Health Care Plan; Facilitates the development of healthy behaviors/lifestyles through preventive screening and assessment programs, patient education and counseling and use of community resources and patient navigators; incorporates cultural backgrounds into teaching process as appropriate
)> Advocates for the Health Center and serve as a liaison to local and State professional societies, organizations etc.; provide presentations as may be needed at Board Meetings, Professional Advisory committees, funding requests, etc.
)> Maintain best practice knowledge level by participation in continuing education opportunities, and utilization of current professional reference materials; maintain continuing education requirements as needed for licensure and board certification; maintain affiliation with and evaluation of teaching programs as needed
)> Provides direct supervision as assigned to Nurse Practitioners to insure the consistent delivery of quality care based on standards of practice, inclusive of safety and infection control practices
)> Adhere to agency policies and procedures and mission driven philosophy

>- Ability to deal with nonverbal symbolism (formulas, scientific equations, graphs, etc.) in its
most difficult phases

CERTIFICATES, LICENSES, REGISTRATIONS
>- Physician licensed to practice in the State of Connecticut
;.. Valid DEA license
;.. BLS and ACLS required
>- Board certified or board eligible in a primary care specialty
;.. Must maintain credentialing requirements for acute care hospital, third party payers, and standards as may be needed by agency accrediting body
» A valid CT driver’s license is also required
