|Screening, Brief Intervention & Referral to Treatment (SBIRT)
SAMHSA is accepting applications for the SBIRT program totaling up to $39.8 million over the next five years. The purpose of this program is to implement screening, brief intervention, and referral to treatment services for adolescents and adults in primary care and community health settings for substance misuse and substance use disorders. SAMHSA expects to fund as many as eight grantees with up to $995,000 per year for up to five years. Applications are due by February 21 at 11:59 p.m. Learn more.

The State of Connecticut Department of Social Services (Department or DSS) is requesting proposals from qualified Federally Qualified Health Centers and Federally Qualified Health Center Look-Alikes (collectively, FQHCs) and Advanced Network Lead Entities (on behalf of Advanced Networks) to become Participating Entities (PEs) in the Department’s Person-Centered Medical Home Plus Program, Wave 2 (PCMH+W2).
|Centering Healthcare Institute: Centering Implementation Plan
The Centering Healthcare Institute (CHI) has launched an aggressive plan to expand access to the Centering model of group healthcare in the communities where group care can have the greatest impact. With this expansion, CHI has developed a program offering technical assistance and implementation services to 130 eligible sites – primarily in FQHCs and community health centers – eliminating the financial barrier to those in resource-limited communities.
