IMPACT is an Excel spreedsheet. The tool was developed by Mark Lyons, MPH, PAC, Deleware Valley Community Health, and Kevin Frick, PhD., Johns Hopkins School of Hygiene and Public Health.

We would like to track who has downloaded the tool so that we can provide you ongoing support including upgrades and modifications. We will also provide you examples of strategies developed by primary care providers to use IMPACT to improve quality of care and to demonstrate cost savings as well as obtain funding for ongoing primary and secondary prevention.
