New England Clinicians Forum
CHCACT is the administrative homeof the New England Clinicians Forum (NECF). The mission of the NECF is to support interdisciplinary health care team members in the delivery of accessible, high quality, community-based health care to all populations with special emphasis on the underserved. Since 2000, the NECF has engaged in programs and education to develop the capacity to conduct research in community health center sites and to support the concept and development of community-based health care as a unique clinical discipline. Another goal of NECF is to advocate for clinicians in their efforts to provide access to high quality community-based care. Community health center medical, dental and mental health professionals from throughout New England count themselves among the over 200 NECF members.
The NECF is a Practice-Based Research Network(PBRN). PBRNs are groups of primary care clinicians and practices working together to answer community-based health care questions and translate research findings into practice. PBRNs engage clinicians in quality improvement activities and an evidence-based culture in primary care practice to improve the health of all Americans. To learn more about the PBRNs visit http://pbrn.ahrq.gov/portal/server.pt.

The National Kidney Disease Education Program (NKDEP) recently developed a series of short web videos that demonstrate sample patient-provider discussions around common chronic kidney disease (CKD) questions. Topics include CKD and risk, treatment, testing and diagnosis, and dialysis and transplantation. To learn more or watch now, visit www.nkdep.nih.gov/professionals/providereducation/index.htm.
The videos are based on NKDEP’s four patient education concepts for providing diabetes self-management education (found at www.nkdep.nih.gov/professionals/patienteducation/keyconcepts.htm).
