Medicaid Outreach Program

Medicaid Outreach

Through a grant with the State of Connecticut Department of Social Services, there are Medicaid Outreach Workers stationed at each Community Health Center in the state during traditional 8:30 AM to 5:00 PM business hours .The goal of these Outreach Workers is to identify eligible families with children and get them enrolled in Medicaid and CHIP, better known in CT as the HUSKY program. Medicaid Outreach Workers also provide referrals to other services a client may need, like food or housing assistance.
