Community HealthCorps
The
Community Health Center Association of Connecticut (CHCACT) is accepting
applications for Community HealthCorps 2011-2012!At this time we are seeking applicants who can serve in Bridgeport, Waterbury, Hartford, and/or Norwich.
Overview:
The
Community HealthCorps program is the largest health-focused, national
AmeriCorps program that promotes health care for America’s underserved while
developing tomorrow’s healthcare workforce. The mission is to improve
healthcare access and enhance workforce development through community health
center sponsored AmeriCorps members. Community HealthCorps members serve for a
minimum of 1,700 hours over the course of 10 months. The 2011 program year is
scheduled to begin no later than November 1, 2011.
Member Activities:
Through
the Community Health Center Association, Community HealthCorps members will be
placed throughout Connecticut at federally qualified health centers (FQHCs). Members will participate in different
activities at different placement sites, ranging from outreach to education to
case management and beyond. Some examples of activities include:
- Bridgeport
– outreach to children at
school-based health centers; lead safety training
- East
Hartford – case management; patient follow-up and referral
- Waterbury
– Smile Builders (school-based dental center) support and outreach; diabetes
case management and patient navigation; performance improvement projects
- New
Haven – coordinate patient groups; develop patient education materials
- Norwich
–addressing client barriers to care; diabetes case management; mental health
case management
- Torrington
– medical case management; maintaining medical records; outreach
*Note: Members cannot choose their placement sites but
geographical preference will be taken into consideration during the selection
process.
Members also participate in required
trainings, conference calls, and monthly meetings, as well as service projects
including:
- Martin
Luther King, Jr. Day
- AmeriCorps
Week
- CT
Mission of Mercy and the Inner-City Dental Mission
- National
Community Health Center Week
- Habitat
for Humanity projects
- Community
clean-ups, health fairs, painting projects, and more
Compensation:
HealthCorps
members enjoy a number of benefits including: - Bi-weekly
living allowance stipend (gross $12,100)
- Health
insurance coverage
- Child
care (if eligible)
- Student
loan forbearance
- Educational
award of $5,550 at the conclusion of service.
Requirements:
An applicant
to the HealthCorps program must:
- Be
a U.S. citizen, U.S. National or a lawful permanent resident.
- Be
18 years or older.
- Have
a high school diploma or a GED, or agree to obtain a high school diploma or its
equivalent before graduating from this program.
- Be
able to commit to a minimum of 40 hours of service per week for 10 months.
CHCACT highly
values applicants who are:
- Bilingual
in English and Spanish.
- Committed
to public health, community outreach, or community health.
- Able
to have access to reliable transportation.
