Children, Youth & Family AIDS Network

| Ryan White HIV/AIDS Part D grantees provide family-centered primary medical care involving outpatient or ambulatory care (directly through contracts or through memoranda of understanding) for women, infants, children and youth with HIV/AIDS. Part D funds the following services: - Family-centered primary and specialty medical care

The Community Health Center Association of Connecticut (CHCACT) receives funding under the Ryan White Care Act Part D to administer the Children, Youth and Family AIDS Network (CYFAN) of Connecticut. This statewide provider network is supported on a contractual basis by CHCACT for medical care, family-centered case management services and outreach. CHCACT fulfills its role in the administration, coordination, program development and reporting functions of CYFAN through day-to-day management of subcontracts with 8 community health centers and 3 hospitals. The eight community health centers that participate in CYFAN all administer comprehensive culturally competent HIV/AIDS outreach services that are driven by case finding and appropriate linkages to care for women, adolescents, youth between the ages of 13-24 and infants.
