Children, Youth & Family AIDS Network

Community Health Center Association of Connecticut (CHCACT) receives funding under the Ryan White Care Act Part D to administer the Children, Youth and Family AIDS Network (CYFAN) of Connecticut. This statewide provider network is supported on a contractual basis by CHCACT for medical care, family-centered case management services and outreach. CHCACT fulfills its role in the administration, coordination, program development and reporting functions of CYFAN through day-to-day management of subcontracts with 7 community health centers and 3 hospitals.  The seven community health centers that participate in CYFAN all administer comprehensive culturally competent HIV/AIDS outreach services that are driven by case finding and appropriate linkages to care for youth between the ages of 13-24.
